- Blepharoplasty
A 54-year-old female presented to Dr. Harbison seeking facial rejuvenation for a heavy, tired appearance around the eyes and upper face. While patients often assume heavy lids are caused entirely by loose eyelid skin, detailed examination revealed a dual concern: redundant upper eyelid skin compounded by significant brow ptosis (brow descent). Accurately diagnosing the brow’s contribution is critical—an anatomical factor many surgeons overlook—because treating only the eyelid skin in such cases often pulls the brow lower, crowding the eyes and leading to unnatural results.
Surgical Approach:
To establish natural periorbital balance, Dr. Harbison performed a combined endoscopic brow lift and conservative upper blepharoplasty. Utilizing small, hidden incisions placed entirely within the hair-bearing scalp, the endoscopic brow lift gently elevated and stabilized the brow arch back to its youthful anatomical position, naturally relieving the downward weight on the eyelids. With the brow properly positioned, Dr. Harbison then performed an upper blepharoplasty to remove only the true excess skin, preserving healthy soft tissue and the native crease.
Results & Recovery:
Shown at just 1 month post-operative. Despite being very early in her recovery phase, the patient displays an open, refreshed, and confident eye area with virtually no visible signs of surgery. Incisions are imperceptibly concealed within the hairline and the natural eyelid fold. By addressing both structural brow laxity and cutaneous eyelid redundancy together, Dr. Harbison delivered an alert, rested look while completely avoiding a "surprised" or over-pulled appearance.
