- Blepharoplasty
A 53-year-old female presented to Dr. Harbison seeking correction for prominent lower eyelid bags, dark under-eye shadows, and crepey lower lid skin that contributed to an exhausted, aged appearance. Her primary goal was to eliminate the under-eye puffiness and smooth the tear troughs while strictly avoiding any change to the native shape of her eyes, a common risk with traditional, aggressive lower blepharoplasty techniques.
Surgical Approach:
Dr. Harbison’s philosophy for lower eyelid rejuvenation centers on structural preservation and volume restoration rather than aggressive tissue cutting. To deliver a completely scarless transformation, he performed a multi-modality lower blepharoplasty:
Transconjunctival Fat Removal: Accessing the orbital fat pockets from inside the eyelid eliminated external skin incisions entirely, preserving the structural integrity of the orbicularis oculi muscle and the delicate eyelid margin.
Micro-Fat Grafting to the Tear Troughs: Autologous fat was harvested, purified, and meticulously layered into the tear trough depression to blend the transition between the lower lid and cheek seamlessly.
Laser Skin Resurfacing: Targeted laser resurfacing was performed on the lower eyelid skin to stimulate collagen remodeling, tighten fine surface lines, and improve skin texture without requiring surgical skin excision.
Results & Recovery:
Shown at 3 months post-operative. The patient demonstrates complete resolution of her lower eyelid bags and tear trough hollows, transitioning into a smooth, refreshed lid-cheek junction. Because the structural anatomy of the lid was preserved and no external incisions were made, the natural shape and cant of her eyes remain entirely unchanged, resulting in a rested, rejuvenated look with zero visible surgical scars.
